Program/Event Request Application

This form is provided for church members and non-church members to submit their recommendations for church events/activities to the program/event committee for review, and to determine if the recommendations will be beneficial to God Glorified COGIC.  Each proposal must be researched prior to submission.  Request must be submitted a minimum of 60 days prior to the event.  All requests must go through an approval process.  The program/event leader will be notified in writing when a decision has been made.
